The key difference is context. IELTS Academic uses texts on general topics — environment, technology, social issues. OET uses healthcare scenarios — nursing handovers, ward rounds, patient consultations, discharge letters. For working nurses already comfortable with clinical English vocabulary, OET often feels more natural and achievable than the abstract writing and reading passages of IELTS Academic.
For Australia via AHPRA: AHPRA accepts IELTS Academic (7.0 in all four bands) or OET (Grade B in all four sub-tests) as equivalent. Many Nepali nurses find OET more achievable because they already understand the clinical context, even if their general academic English falls below IELTS 7.0. The OET Writing sub-test asks you to write a referral letter or discharge summary — tasks practising nurses do regularly.
For UK via NMC: NMC accepts both IELTS Academic (7.0 in all bands) and OET (Grade B in all sub-tests). The choice depends on your strengths: if you struggle with academic reading texts (IELTS) but are comfortable with clinical communication (OET Listening and Speaking), choose OET. If your general English is strong but medical vocabulary is limited, IELTS may be easier.
For nursing programmes (study): Most universities in Australia, UK, and Ireland accept both IELTS and OET for admission to nursing degree programmes. OET is not required or preferred for student admissions — IELTS is the standard. OET is primarily relevant for nurses seeking professional registration, not students applying for degree programmes.
Bottom line for most Nepali nurses: If you are a practising nurse with good clinical English and have struggled with IELTS Academic, try OET. If you are a nursing student or your general English is strong, IELTS is simpler and more universally accepted. Both require the same overall proficiency level — they just test it differently.
OET Sub-Tests — What Each Involves
Listening (45 minutes): Three parts — a consultation between a healthcare professional and patient (Part A), healthcare professional presentations or interviews (Part B), a healthcare professional presentation (Part C). For nurses, Part A involves completing notes from a nurse–patient consultation. Questions include form-filling, multiple-choice, and short-answer. Grade B requires approximately 70%+ accuracy.
Reading (60 minutes): Two parts — Part A is an extended text (3,000 words) on a healthcare topic with summary-completion and true/false/not-given questions. Part B is three shorter texts with multiple-choice questions. Content is healthcare-specific but not hands-on clinical — you might read about infection control protocols, mental health policy, or nursing research. Grade B requires approximately 65%+ accuracy.
Writing (45 minutes): Write a letter of 180–200 words — typically a referral letter from a nurse to a specialist, or a discharge summary letter to a GP. You are given case notes and must select relevant information and write in appropriate professional healthcare English. Nurses with clinical writing experience have a clear advantage over general English students. Assessed on: purpose, content, conciseness/clarity, genre/text type, organisation, and language.
Speaking (20 minutes): A role play with an OET examiner (acting as a patient, carer, or colleague). You will have two scenarios. In each, you are the nurse — the examiner plays the patient or family member. Tasks might include: explaining a diagnosis to an anxious patient, taking a medication history, or discussing discharge instructions. Assessed on: intelligibility, fluency, appropriateness of language, resources of grammar and expression, and engagement.
Grade B — What It Actually Means
OET uses a 0–500 score scale converted to a letter grade (A–E) for each sub-test. For professional registration with AHPRA (Australia) and NMC (UK), you need Grade B (score ≥ 350) in all four sub-tests: Listening, Reading, Writing, and Speaking. There is no averaging — you must reach Grade B separately in each sub-test.
Grade B is roughly equivalent to IELTS 7.0 (each band). In practice, nurses who score 7.0 across all IELTS bands tend to score Grade B across all OET sub-tests — and vice versa. The difference is that for nurses with strong clinical communication skills, OET's healthcare-specific scenarios make Grade B more achievable than IELTS 7.0 in academic reading and writing.
If you miss Grade B in one sub-test, you can retake that individual sub-test within 24 months of your original test. Sub-test retake fee: USD 105. This makes OET more forgiving than IELTS if you are close to the required level in most sub-tests but need an extra attempt on one.
How to Book OET from Nepal

Test dates are typically offered 1–2 times per month. Registration opens approximately 6 weeks before the test date.
Booking process: Create an account at oet.com, choose your test date and location, pay the fee online (credit/debit card or bank transfer). Total fee: USD 275 (approximately NPR 37,000–39,000). You will receive a Candidate ID and confirmation email. Bring your original passport on test day — no other ID is accepted.
Results are released online within 16 business days after your test date. Log in to the OET Candidate Portal to download your Statement of Results, which you submit to AHPRA or NMC. Statements are valid for 2 years from the test date for most registration bodies.

For the Writing sub-test: Practice writing referral letters and discharge summaries daily. Use real hospital case notes as practice material. Focus on: correct salutation, clear reason for referral, relevant history summary, medication list, and professional closing.
